Allow at least 48 hours if your prescription does not have refills. In an emergency situation we may be able to advance you some medication.

2. Doctor’s Name
3. Name and phone number of the pharmacy where the prescription was last filled
4. T&F Pharmacy location where you would like your prescription(s) filled
